Currently, when using Nihilus’s annihilate, should i not regain health? I used to… For example i was on red health, used annihilate on someone with perfect health, but i regained nothing, then was defeated.

no you dont gain health from it, you just gain max health. say you had a nihilus with a max HP of 100, but currently has 10 hp left, and you annihilate someone that also has 100 max HP. now your nihilus will have 200 max HP but should still have 10 hp.
I'm pretty sure that when you gain max health, you also gain that much health, the bar just doesn't appear to change because both numbers increase at the same time so your percentage missing is still the same.
I think you gain health based on your % of HP remaining, so you stay on the same % of max health after the annihilate. So if you had 10/100 HP (10%), after you would be at 20/200 HP (10%), and because bars are based on % HP it looks like you didn't gain any HP.

I would agree with that but General Skywalker makes me question it. when he reduces max health he can make it look like his damage heals you since the HP bar fills up the bar as you lose max health. (from 70/100 HP to 70/80 HP) so if it also equalized to the correct percent we would never "see them heal" after the attack
I think it comes down to how the gain or loss happens.
Let me say DN is most likely WAI as he has been in the game a long time and this has been brought up a lot when he was released
Explanation below is based on what I have seen in the game.
Current health/protection -> Top
Max health/protection -> bottom
% gains
Both top and bottom will go up by the amount gained
1000 with 10% gain
Top and bottom go up by 100
This can be seen as it is actually healing the unit and can be seen with units like KRU regaining protection under SLKR lead when he is getting a lot of TM removal in the assault battle.
This can be also seen from EP zeta where he gains max protection and had 0 on top and suddenly restores some
% reduction
Bottom goes down and top only goes down if it is more than the bottom
This can make it look like the unit is healing
1000 bottom current 900 10% reduction
900 top and bottom
1k top and bottom 10% reduction
900 top and bottom.
Example is GAS crit hitting unit with no protection
Flat gains
Bottom goes up by amount gained an top is adjusted to be same % of bottom as it was before.
DN annihilate.
10k bottom 5k top
annihilates unit with 10k bottom
Now at 10k top and 20k bottom
This is why I think it looks like it isn't working, you can see by a heal after that his max health definitely went up.
I also believe it is like this because in the scenario above if he gained the max health on top he would be at 100% health and I have never seen the color bar change always looks the same.
