How and when to add your DOB and Name

I am unable to access guild chat because somehow EA thinks that I am underage. I have posted on their support forum and was told that this cannot be fixed, and that I need to start a new game.

So I tried setting up a new account. I paid close attention to each screen of the tutorial looking for the time and place to enter my correct DOB and name (not Mol Eliza). I played for over an hour and got to level 15 but at no time did it ever ask me for my name and date of birth.

So could someone please tell me where and when you are supposed to add your character name and date of birth?
