- Contacted Google support, confirmed order went through, did some basic troubleshooting at their request - clearing cache on Google Play and Google Services apps and restored purchases in-game.
- Contacted EA support, three days later their response:

That's crazy. All we want is what we "paid" for.
I guess this is good for anyone else who is wondering exactly how to redeem this promotion when it works correctly.
Still do not have what I paid for though.
