Fleet Rewards Missing!

I have tried to fill-in the form, again the Submit button just don't proceed to anything. And to be able to see the preview, the information should be completed first. See screenshots of proof below:

Yeah sorry, I thought we had a fix and moved it to bugs where it is archived and can't be commented on.

I flagged it and hope to get you some help soon.

@Kyno Tried EA Help earlier, and after a couple of hours, my case was closed without any reply or notice.. what!!!???
